The Service Coordinator is responsible for coordinating individualized plans & services, managing budgets, and overseeing medical care for individuals living in their own apartments. The service coordinator schedules, transports, and supports individuals in medical appointments. The service coordinator must be focused and detail oriented in their approach to their work. The service coordinator writes the service plans and develops training/essential lifestyle goals with individuals. It's a busy, diverse, and interesting job! The agency thoroughly prepares and trains new employees to complete the tasks of a service coordinator. Key Qualifications: Exceptional time management & organizational skills Computer Skills (Microsoft Word, Excel) Money management skills (bank accounts, stretching a dollar, etc) Understanding of basic concepts for shaping behaviors Good writing skills & effective verbal communication Ability to advocate for/with individuals w/ medical community, families, and outside agencies.

The schedule is: Monday, Wednesday and Friday 7a-3p & Tuesday and Thursday 10a-6p

The Enfield team is seeking a reliable, dedicated, and kind individual to fill a full time support assistant position. The team is located in Norfolk off of West Little Creek Road. The support assistant provides direct care to adults with cognitive disabilities living in their own apartments in the community. The individuals receiving support with day-to-day tasks are adults with a wide variety of needs and interests. The support assistant helps with many tasks including meal preparations, grocery shopping, laundry, medications, recreation, and hygiene. Individuals may use wheelchairs and require assistance with lifting and transferring. The support assistant is one of approximately twenty staff persons working as part of the team. We are looking for a motivated individual who works well as part of a team and takes the initiative to get things done. In addition to the annual salary, other benefits include health insurance, dental insurance, and generous paid leave.

The schedule is: Sunday 7A-3P, Monday, Tuesday and Wednesday 1P-9P, and Thursday 7A-3P

Part Time Direct Care Support Staff

Hope House Foundation is currently accepting applications for part-time direct care support staff. The support staff position is rewarding, challenging, and fun! There are 14 different service teams located in Virginia Beach, Chesapeake and Norfolk. We are looking for professional people willing to go the extra mile with a friendly demeanor and caring heart.

Hope House Foundation is currently hiring part-time employees to work evenings and weekends. Our support staff work directly with the people receiving services assisting with all activities of daily living. Support staff assist individuals with preparing their meals, maintaining their apartments, completing laundry, medical appointments, recreation and socialization, personal hygiene and any other tasks that the individuals may desire assistance. Hours available on weekdays are generally 4:00 p.m. to 10:00 p.m. shifts. We have various shifts open on the weekend in the mornings, afternoons, and evenings. Support staff are matched with a specific location/ team and develop a regular schedule with their supervisor. A one year commitment is preferred. Support staff must be able to work a minimum of two shifts per week plus attend a monthly team meeting. Community Support Coordinator

The agency is seeking a reliable, energetic, and resourceful person to fill a Community Support Coordinator position. Leadership abilities and effective communications skills are needed in this role.

The open position is located in the City of Chesapeake in the Deep Creek neighborhood at our newest team called Lake Village. The Community Support Coordinator develops links and makes connections in the community that support true integration and community participation for the 8 people receiving services from Hope House Foundation at this location. Duties include but are not limited to: The development of inclusive activities and natural supports for those individuals supported by Hope House Foundation, securing knowledge of local clubs, culture and neighborhoods in order to better plan and achieve integration. To assist with activities of daily living including but not limited grocery shopping, apartment maintenance, laundry, personal hygiene, and recreation. To serve in a leadership role in the team in terms of ensuring services are being delivered properly, helping staff problem solve service needs, and assisting with scheduling issues when the supervisor is not at team in the late evenings and on weekends.

The schedule is: Deep Creek/Chesapeake- Tuesday through Saturday 1p-9p flexibility required.

Support Assistant ONR

Hope House Foundation is currently accepting applications to fill a full time overnight support assistant position. Hope House has teams in Norfolk, Chesapeake, and Virginia Beach. Employees work out of a staff office apartment in the apartment building or complex where the individuals receiving support are living. Employees are hired to work with a specific team/location. The overnight assists with a variety of duties including overnight supervision, office tasks, cleaning tasks, and morning support.

The current opening is at our newest team called Lake Village located in Chesapeake. The team will eventaully support approximately 8 individuals with developmental disabilities. Individuals live in apartments located near one another. The overnight support assistant provides direct care with day to day tasks including laundry, apartment maintenance, medications, safety checks, breakfast preparations, dressing, and hygiene. The overnight staff person assists with office tasks such as filing, documentation checks, and light housekeeping of the common areas. We are looking for a responsible person who will be awake and productive during the overnight hours.

The schedule is: Sunday through Wednesday 10p-8a
